In order for an employer to conduct a New Hampshire criminal background check on one of their employees, they will need to obtain their employee’s authorization by having them complete the New Hampshire Criminal History Request Form. This form will then be mailed, along with a $25 fee, to the Criminal Records Unit. There are few restrictions on an employer’s ability to inquire after their employee or applicant’s criminal history. However, background checks may not be conducted in a way that is either an invasion of privacy, or discriminatory or defamatory in its implementation.

Step 1 – Download/Print History Request Form

First, you will need to obtain the New Hampshire Criminal History Request Form, which you may either fill out on your computer or print and fill out by hand.

Step 2 – Fill Out Section I

In Section I, the employee will need to provide the following information:

  • Name
  • Address
  • Date of birth
  • Hair color
  • Eye color
  • Sex
  • Driver license number
  • State

Below that, they should indicate the “Purpose of Record” and provide their handwritten signature and the date. In the fields below that, the employer will put their signature and the date.

Step 3 – Fill Out Section II

Below Section II, provide the “Name of the Person/Entity to Receive Record,” the mailing address for the recipient/employer, and the subject/employee’s signature and date. This form must be validated by having a Notary Public provide their signature, the date of signing, and their seal.

Step 4 – Attach Payment and Send Completed Form

If you have included a self-addressed envelope, check the appropriate box. Next, if you have a prepaid account, check the appropriate box and enter the account number. Attach your check or money order for $25 and mail the complete package to: Criminal Records Unit // 33 Hazen Drive, Concord // NH 03305.
