Louisiana criminal background check law states that only employers may request criminal history information from the Louisiana State Police (LSP) database. Furthermore, criminal records will only be provided once a signed consent form has been submitted. There are a number of departments/agencies (§ 15:587 A.(1)) who are entitled to criminal history information and exempt from the requirement of obtaining consent prior to making the request. The general public cannot request criminal history information on anyone but themselves. There are two (2) ways for employers to complete a criminal background; they can do so online, or by mail. The online option is faster but may yield inaccurate results, as the search is name-based instead of fingerprint-based.

Background Check Authorization Form

Most employers are required to obtain signed consent from applicants before conducting a criminal background check. The applicant’s signature must be included on the request forms before they are submitted. An employer may choose to ask applicants to submit a Background Check Authorization Form as well to keep for their records, but this is not mandatory.

Criminal Background Check – Mailed Request Forms

Step 1 – Access LSP Website

All request forms and disclosure forms can be found on the Bureau of Criminal Identification and Information section of the LSP website.

Step 2 – Authorization Form

Select the Authorization Form to submit a request for an individual’s criminal records. The form must be printed and completed in ink only. The bottom section of the form must be signed by the applicant in order to authorize the dissemination of their criminal history information. Fingerprints should be submitted for a positive identification.

Step 3 – General Disclosure Form

The General Disclosure Form must be submitted with the authorization form when an employer is conducting a background check on a potential employee.

Step 4 – Mail Forms

Forms and the $26 fee (check or money order) should be mailed to the following address:

Criminal Background Check – Online

Step 1 – Navigate to LSP Website

To perform an online criminal background check, go to the LSP Internet Background Check webpage. Registered users can click on the Sign In link and new users can create an account by clicking Register.

Step 2 – Login

Registered users can enter their “User Name”, “Password”, and “Agency Code” into the appropriate fields. Once logged in, a criminal history search can be performed.

Step 3 – Register

Individual’s wishing to create a new account must sign the terms of use before continuing.

Enter the following information to sign up for an account:

  • Agency name
  • Tax ID # or Facility License #
  • Business type
  • Physical address
  • Mailing address
  • Contact information

To finish the enrollment process, provide the following information into the empty fields:

  • Full name
  • Date of birth
  • Drivers license number
  • SSN
  • Race
  • Sex
  • Physical address
  • Mailing address
  • Contact information

Once the account information has been verified, registered uses can begin their criminal history information search.
