A Connecticut criminal background check can be requested through the Department of Emergency Services and Public Protection (DESPP). In order to obtain an individual’s criminal record history, a Criminal History Record Request Form (DPS-0846-C) must be filled out and submitted to the State Police Bureau of Identification (a division of the DESPP). Criminal background check requests are either name-based or fingerprint-based. Fingerprints must be completed by a law enforcement agency or by the DESPP, subject to a $15 fee.
Cost – $36 Name/D.O.B. Search (will only yield a “yes” or “no” reply); $50 Complete Criminal History Search by Name or Fingerprint; $65 Complete Criminal History Search by Name or Fingerprint (fingerprinting completed by DESPP)

Step 1 – Background Check Authorization Form
Before conducting a criminal background check, employers might consider having the employee/applicant submit a Background Check Authorization Form to avoid any future complications concerning lack of consent.
Step 2 – Access DESPP Web Portal
Criminal reports and records information can be found on the State Police Bureau of Identification webpage on the DESPP website.
Step 3 – Download Criminal History Record Request Form
The request form can be found near the bottom of the page, marked “Form DPS-0846-C“.
Step 4 – Complete Form
Enter the following information into the appropriate fields on Form DPS-0846-C:
- Requester name
- Requester address/city/state/zip
- Requester phone number
- Requester email address (for results by email)
- Subject’s full name
- Subject’s date of birth
- Any aliases/maiden names/birth dates (if applicable)
The appropriate fee must be attached to the form in the form of either a check or money order made out to the Treasurer-State of CT. Form and payment should be mailed to the address provided on the form.
